Sheffield Accountable Care Partnership

Primary Care Sheffield is a member of Sheffield’s Accountable Care Partnership (ACP), a partnership of seven the city’s health and care organisations working together to share responsibility for planning and delivering all health, care and wellbeing services.

The partners are: Sheffield City Council, NHS Sheffield Clinical Commissioning Group, Sheffield Children’s NHS Foundation Trust, Sheffield Teaching Hospitals NHS Foundation Trust, Sheffield Health and Social Care NHS Foundation Trust, Primary Care Sheffield Ltd and Voluntary Action Sheffield.

Dr Andy Hilton, chief executive officer of Primary Care Sheffield sits on the ACP Board representing primary care.
