The Primary Care Sheffield paramedic home-visit service can be accessed by GP practices in Sheffield for patients with acute minor illnesses and injuries.
The aim of the service is to provide people with appropriate care in their own home, and to reduce the current pressure on primary care and other health and care services.
Information for practices
How can practices refer a patient?
Slots will be released daily, and will be available on the Fairlawns hub in the same way you book extended access and ANP appointments. The rota will be labelled as VISITS.
The Paramedics will not all be Emergency Care Practitioner’s. Therefore to refer, practices need to ensure the patient has been triaged by a GP. Those that have not been triaged will be rejected and passed back to practices.
The visits will occur throughout the day, so please indicate in the booking if there is any urgency or times when a visit may be more suitable.
We also need to be made aware of any details about entry e.g needing to knock loudly.
Follow up with your practice following the visit
The service is overseen by a GP which removes the need for practices to provide clinical oversight or to be involved in prescribing. If practices want us to update them following the visit please let us know, otherwise the episode will be completed by the team.
